CLAIM BY WORKER's FAMILY

1. Who can claim? Workers who were employed by or lived within the vicinity of the mines owned by Kuruman Cape Blue Asbestos (KCB) or Danielskuil Cape Blue Asbestos (DCBA) between January 1952 and 30 September 1981, and who have been diagnosed with a compensable asbestos related disease.

2. How do I claim? The worker collects his/her ID , Work Record and obtains a medical letter to go to a doctor.

3. What happens next? The worker is sent to a doctor for medical examinations and the medical records are sent to Cape Town for a diagnosis.

4. What happens next? The workert returns to the doctor. The doctor explains the diagnosis to the worker. If the worker has an asbestos related disease, then he/she lodges a claim.

5. How is the claim followed up? A Claims Form is completed. A Bank Account is opened. These documents are sent to the Johannesburg Office.

6. And then...? The Johannesburg Office processes the form. The award is calculated. The claimant returns for further consultation.

6. Finally... The claimant signs the Release and Discharge Form. The claimant is paid.
